3. Eligibility for Services

You must be at least 18 years of age to use our services or access our Site. If you are seeking services on behalf of someone else, you represent and warrant that you are authorized to do so. Eligibility for specific services may require an assessment or referral from a healthcare provider, or qualification through the Medicaid waiver programs, such as the iBudget program for Agencies for Persons with Disabilities.

4. Client Responsibilities

• Communicate any changes in the client’s health or care requirements promptly.

5. Payment and Billing

The services provided by St. Levon Haven, LLC are funded through Medicaid waiver programs, including the iBudget program for individuals with disabilities. Clients eligible for this program will have their services covered as specified in the waiver.

No copays or deductibles are required for services under the Medicaid waiver program. Clients or their representatives are responsible for ensuring that eligibility for Medicaid waiver funding is maintained and for promptly notifying St. Levon Haven, LLC of any changes to eligibility or funding status. Services rendered outside of Medicaid coverage may require additional fees, which will be discussed and agreed upon in advance.
