Q: Who do you serve?
A: We serve individuals with disabilities across Northeast Florida, including Jacksonville and surrounding areas. Our goal is to empower every client to live as independently and confidently as possible.

Q: What types of services do you offer?
A: We provide Personal Supports, Life Skills Development 1: Companion, and Respite care services—all tailored to the individual’s needs.

Q: What age groups do you serve?
A: We work with adults with disabilities. Each plan of care is customized to the individual’s needs and goals.

Waiver & Payment

Q: Do you accept the Florida iBudget Waiver?
A: Yes! St. Levon Haven is an approved provider under Florida’s iBudget Waiver Program. We proudly support clients funded through Medicaid.

Q: Do you accept private pay clients?
A: At this time, we primarily accept clients through the iBudget Waiver. However, we’re happy to discuss private pay options if needed.

Getting Started

Q: How do I refer someone for services?
A: You can refer someone by visiting our Referral Program page or contacting us directly. Waiver Support Coordinators, families, and other professionals are welcome to submit referrals.

Q: How soon can services begin?
A: Once we receive a referral and complete the intake process, services can usually begin within 1–2 weeks, depending on the client’s needs and location.
