General Questions

Q: Who do you serve?
A: We serve individuals with disabilities across Northeast Florida, including Jacksonville, Orange Park, Middleburg and St. Johns, FL and surrounding areas. Our goal is to empower every client to live as independently and confidently as possible.

Q: What types of services do you offer?
A: We provide Personal Supports, Life Skills Development 1: Companion, Respite care, Supported Living Coaching, Homemaker and Companionship services—all tailored to the individual’s needs.

Q: What age groups do you serve?
A: We work with adults with disabilities. Each plan of care is customized to the individual’s needs and goals.

Waiver & Payment

Q: Do you accept the Florida iBudget Waiver and CDC+ Waiver?
A: Yes! St. Levon Haven is an approved provider under Florida’s iBudget Waiver and CDC+ Program. We proudly support clients funded through Medicaid.

Q: Do you accept private pay clients?
A: Yes, we’re happy to discuss private pay options if needed.

Q: Do you accept insurance?
A: Yes, we accept insurance. Please contact us to verify coverage and discuss your plan details.

Getting Started

Q: How do I refer someone for services?
A: You can refer someone by completing our contact formor by contacting us directly at 904-820-6865. Waiver support coordinators, case managers, families, and other professionals are welcome to submit referrals.

Q: How soon can services begin?
A: Once we receive an authorization and/or a referral and complete the intake process, services can usually begin within a 1 day to 30 days, depending on the client’s needs, location and authorization.
