    Who is eligible for housing transition support through St. Levon Haven?

    Housing transition support is available only to clients who are currently authorized for Supported Living Coaching (SLC) services through APD. Services are provided to clients transitioning into a supported living setting or moving between supported living settings.

    Is there a cost for housing transition support?

    Housing transition support is included in the client’s approved SLC plan. No out-of-pocket cost is required. Individuals not authorized for SLC services are not eligible for housing transition support.

    What does housing transition support include?

    Services may include:

    • Assistance identifying appropriate supported living options

    • Coordination during the transition process (once provider is certified)

    • Guidance with housing applications and documentation

    • Move-in planning support

    • Collaboration with case managers and other service providers

    All support is tailored to the client’s needs and eligibility.

    Do you provide direct housing or own properties?

    No. St. Levon Haven does not own, lease, or manage residential properties. We provide guidance and coordination only.

    Can a new agency help locate housing before they are certified for SLC?

    Yes, in a preparatory, informational capacity only. This may include identifying property owners or homes that are safe and accessible.

    Important compliance points:

    • Preparatory, informational only – No official SLC services are provided.

    • Cannot provide official SLC housing transition services until certified – Tasks like selecting a home, coordinating a move, or completing documentation must wait.

    • WSC oversight required – All preparatory activities must be directed and supervised by the client’s Waiver Support Coordinator (WSC).

    No billing or official SLC service occurs during this phase. This allows clients to explore housing options safely while remaining fully compliant with APD rules.

    Do families or guardians stay involved during the transition process?

    Yes. Families and guardians are encouraged to participate throughout the transition to ensure the client’s needs, preferences, and safety remain the top priority.

    Does St. Levon Haven receive referral fees, commissions, or placement incentives from property owners?

    No. St. Levon Haven does not receive referral fees, commissions, incentives, or any form of compensation from property owners, landlords, or housing providers. Housing recommendations are made solely based on the client’s needs and preferences.

    Does St. Levon Haven own, lease, or manage residential properties?

    No. St. Levon Haven does not own, lease, operate, or manage residential properties. We are not a housing provider.

    Is housing transition support billed separately to APD or other funding sources?

    No. Housing transition support is included in the client’s approved SLC plan and is not billed separately.

    Does St. Levon Haven have financial relationships with landlords or property managers?

    No. St. Levon Haven does not maintain financial partnerships, shared ownership interests, or compensation agreements with landlords or residential providers.

    What is St. Levon Haven’s role in housing placement?

    St. Levon Haven provides support, guidance, and coordination to eligible clients transitioning into or between supported living settings. Final housing decisions are made by the client and/or their legal representative.
