CDC+

We provide CDC+ services through a program under Florida’s Agency for Persons with Disabilities (APD) that empowers individuals with developmental disabilities to self-direct their care. This program gives participants the flexibility to choose their own providers and manage a monthly budget to meet their unique needs, promoting independence, control, and personalized support.

CDC+ (Consumer Directed Care Plus)

  • Life Skills Development 1: Companion Services

    Life Skills Development 1: Companion support, helps individuals build essential daily living skills while increasing independence. The program emphasizes non-medical assistance, including supervision, social engagement, and support with daily routines, while promoting safety and confidence in both home and community settings. Participants are encouraged to take part in social and recreational activities and may be accompanied on outings either one-on-one or in small groups of up to three adults, fostering meaningful connections and active community involvement without the inclusion of hands-on personal care.

    Supervision – Monitoring and oversight to ensure the individual remains safe and supported in their environment.

    Social Interaction – Encouraging conversation, companionship, and relationship-building to reduce isolation and promote emotional well-being.

    Support for Daily Routines – Assisting with structure and consistency in everyday tasks such as scheduling, reminders, and general organization.

    Community Participation – Helping individuals engage in community activities, events, and outings to build confidence and independence.

  • Supported Living Coaching Services

    Supported Living Coaching empowers adults with intellectual and developmental disabilities to live as independently as possible. Coaches provide personalized, hands-on guidance in each person’s home or apartment, focusing on everyday life skills such as finding housing, managing finances, planning meals, and coordinating medical care. The program also encourages active community involvement, supporting participation in local events, recreational activities, and employment or training opportunities.


    Household Management
    Guidance and support in maintaining a clean, safe, and well-organized home environment.

    Money Management
    Help with budgeting, paying bills, managing bank accounts, and making informed financial choices.

    Health Management
    Assistance with medication routines, scheduling and attending medical appointments, and promoting overall well-being.

    Community Inclusion
    Support in accessing community resources, engaging in activities, and building meaningful social connections.
